Operasjon av urinveiskreft i nyrebekken og / eller urinleder med lymfeknutekirurgi

Vi fjerner lymfeknuter retroperitonealt ut med cave og aorta i følge templater i samband med nephroureterektomi ved urotelialcancer i øvre 2/3 av ureter og nyrebekken. Dette for å kartlegge om det er de rette lymfeknuterne vi fjerner og om det øker overlevelsen.

Om studien

Two out of three tumours in the upper urinary tract are located in the renal pelvis (1). Muscle-invasive urothelial carcinoma is probably more common in the upper urinary tract, these tumours represent approximately 45 % of renal pelvic tumours (2, 3) compared to 25 % of urothelial carcinomas of the bladder. As in the bladder, lymph node metastases are rare in non-muscle invasive disease. Information regarding indications, extent and possible curative potential is currently lacking for lymphadenectomy in conjunction with nephroureterectomy for urothelial carcinoma in the upper urinary tract (UUTUC). Today we excise lymph nodes that are enlarged or suspicious of metastasis, but not systematically.

There are however retrospective series with survival data for patients with lymph node metastasis that report long term survival after surgery as monotherapy (4) with similar survival proportions as in bladder cancer with lymph node metastases after radical cystectomy. A retrospective study from Tokyo (5) was expanded to the only available prospective study, where 68 patients with UUTUC were submitted to template-based lymphadenectomy (6).


A larger prospective lymph node mapping study would give a substantial amount of information regarding location of lymph node metastases and risk factors associated with lymph node metastases.

In regi´ of the current study we will now perform these operations with standardized extended lymph node dissections utilizing an internationally accepted template. This change will lead to a more extensive lymph node dissection.

Study design: Prospective template based lymphadenectomy in patients with clinically muscle-invasive UUTUC in the renal pelvis or upper 2/3 of the ureter.
